Chapter Membership Application

To create a Crochet Guild of America (CGOA) Chapter:

• You must be an active CGOA member first
• Do not be logged into the site as yourself
• That is, log out from the Member Portal 
• Then apply to be a chapter

1. Select 'Chapter'
2. Enter chapter president info:
   • First name
   • Last name
   • Email
3. Enter chapter name as 
   • Organization name
4. Enter chapter location info:
   • City
   • State
   • Meeting info
5. Enter payment info

Fields with a red asterisk are required. 

Do not choose "Invoice Me" (application will not be processed)
